For your convenience, new patients may download the registration paperwork and complete them prior to arrival for the first day of therapy. Please print and bring them with you, or FAX to 402.939.7940. (Due to security/privacy issues with email, please do not send completed documents, containing sensitive information back to us via email).
If paperwork is not completed prior to your arrival, please arrive for you appointment 20 minutes early to allow time for completion at the clinic. We look forward to seeing you!
